Eligibility Requirements

The Good Samaritan Wildflower Clinic provides free medical and dental services to St Johns County residents. To qualify, patients must not have insurance and must demonstrate St Johns County Residency. We can only serve patients who are over the age of 18.

Dental Services include Exams/X-rays, Extractions, Fillings, and Hygiene appointments.

Medical Services begin with a Primary Care office visit, where lab work and referral services can be ordered at no charge where available.

All dental Fillings and Extraction/Exam clinics as well as medical Primary Care clinics function on a first come-first serve basis. Because only a limited number of patients can be seen each day, we recommend arriving early.

We do not provide emergency services. If you are experiencing a medical emergency such as chest pain or trouble breathing, call 911 immediately.

What to bring with you to your visit:

  • Please bring a photo ID and proof of residency in St Johns County. Proof of residency may include your Drivers License or Photo ID, utility bill, or other mail at your current address with your name.
  • Please bring any pertinent medical records. You may have them faxed to us at 904-824-1718.
